Web Forms :: Create Email Form Where User Enters Specific Email

I need to create a form that would work like this Choose Provider :( user would select gmail, yahoo, hotmail, ext) Enter Name: ( user enters there name) Message : ( user enters message) and then the "submit"button.
I am new to ASP.net but i used VB a lot back in days, i am working on a site and i want to create form so the data from it could be submited to the email. i have put some stuff together but need some corrections. It feels like having brain shut down.

I would like to know if there is any free control to create a contact us form with send mail functionality. I've seen AutoEmail which is not free. Is there any free control?.
i have very big form in my page (alot text boxes some chkboxes abd more. after the user fill his form he click a submit button. after that i want to get all the form (i mean with all the style and exactly the way it look ) so i have a function that i have build to send mail. i want the msg body will be the form how can i do that? just to get the form and get it as a string (with values )

I have to make a login form using vb.net. The login form should either accept
Username ID or
Email ID or
Phone number
---- either of one ,and match it with the password of the corresponding record in the user table.
The user table has following column:- U_ID(varchar),mail(varchar),Phone(Number),Password(Varchar)
The form has two textfields - 1. Username
2.Password
And a submit button. If combination is correct I should be redirected to the Index page.
But I don't have to scan through all the columns of the user table to match the password.
(a)If the user enters user_id say- ID 10012 then we should be able to search only the record in the user_id column as it has alphanumeric or varchar data type and match with corresponding password.
(b)If the user enters Email id we should be able to search through the email id column as email id contains "@ and ." sign.
(c)If the user enters phone number then we should search only the phone column as it has numeric datatype.
After a user creates their account with the create new user wizard, I would like to have a confirmation email sent that requires them to click on a link in the email to confirm their account and verify their email address before account is activated.
There has to be a simple way to send a confirmation email to new users. Every article that I've read really doesent tell me how to use the sendingmail event. In addition when I had it working, it wouldnt send emails to places like gmail or aol.

I was able to find examples throughout the internet that more or less accomplished what I needed to be done, but now I have run into some problems. I need to be emailed the form data when the users submits it and then the user needs to be redirected to a thank you page, in this case "thanks.asp". I have been unable to get the form to redirect users to the thanks page, and in my efforts I think I might have messed the email process up as well.
I have create user wizard control on my page as below with mail definition setup to send a welcome email to new registerd user.

This one worked and I got welcome email. The setup in Web Config is correct. Is it because I have the oncreateduser="CreateUserWizard1_CreatedUser" or some other conflict issue.
In my registration form, there is field to enter email-id of member, i want to restrict user, so that he would not able to enter gmail, yahoo, radiff.... emailids is this possible.., Only company email id eg: priyanka@juncturetech.net must be allowed?
I have C# web application that I want it to send out an email on a certain date, such as if equipment isn't returned on the day it was due. Data is in MS SQL database.
I've got all the email code already done, and I'm just curious how do I trigger this email, say after the COB on that particular day?
